Clinicians at Ainsdale Medical Centre may request that you provide samples to help with diagnosis or management of health conditions.
Please make sure that all samples that are to be brought to the surgery are received before 12:00 noon with your full name and full date of birth clearly on the label attached to the container. Without this information we are unable to accept your sample. Containers can be obtained from the reception desk. Any specimens not in sterile containers will not be sent for analysis as they can become contaminated. Please see the images to make sure you have the correct container.
You can now have blood tests taken at community clinics, and the results will be sent to the clinician who has requested them. Please make sure you take your form with you.
